COVID Screening System
Option 1 - For those with smart devices + access to data (either through your phone plan or site WIFI)
- Arrive to site at dedicated entrance/entry point
- Using your device (phone/tablet), open the Camera application or a QR Code scanning application to scan QR Code. A pop up notification should appear that takes you to the screening web page.
- Complete the screening questionnaire
OR
- Open your phone/tablet’s browser and go to covidcontrol.ca and enter the 6 digit location code found on the QR poster.

If the screening is a Pass, a green badge will be produced. Requirements to show badges will vary based on location and program.
If the screening is a Fail, a red badge will be produced and Contract Tracing will be notified. Access to the building will be denied.
Option 2 - For those without smart devices and/or have no access to data
- Arrive to site at dedicated entrance/entry point
- Inform staff currently on site that you have not completed a screening.
For staff conducting the screening:
- Using your own device or an agency provided one (laptop/tablet/phone/computer) proceed with the instructions provided in Option 1 above.
- Rather than entering your own information into the system, input the staff/client/vendor/visitor’s information and survey answers.
- If devices are unavailable, printed copies of the screening can be completed however they must be entered into the COVID Control database immediately after staff/client/vendor/visitor entry into the building.
Option 3 - Advanced Screening (must be completed no more than 3 hours before arrival on site)
- Visit covidcontrol.ca and enter the 6 digit location code
- Complete the screening
- Save the screening badge
